Service Description
This annual membership is available for all clients only that are over the age of 18 years old, who have a valid U.S. Driver's License. It includes an Initial Consultation Appointment for 60-minutes, Monthly Office Hour Group Meetings over 12-months, and 3 Follow-Up Appointments for 60-minutes. It does not include any testing or products. This annual membership fee of $1,499 is self-pay only and not reimbursable with insurance. All sales are final. *The term "Family" includes up to 5 individuals living in the same permanent residence within the United States.
