Service Description
This annual membership is available for all clients only that are over the age of 18 years old, who have a valid U.S. Driver's License. It includes an Initial Consultation Appointment for 60-minutes and Monthly Office Hour Group Meetings over 12-months. It does not include any testing or products. This annual membership fee of $999 is self-pay only and not reimbursable with insurance. All sales are final.
